Does my child need to be registered with Easter Seals to apply for Recreational Choices funding?
Yes. Please note that the Incontinence Supplies Grant Program is a separate registry. To be registered with Easter Seals Ontario, your child must have a long term physical disability that restricts independent mobility and requires he/she to use a primary mobility device (such as a wheelchair, walker, braces, AFOs, etc). Eligibility does not extend to children with a primary diagnosis of developmental disabilities such as autism.
What age of child is eligible?
Between the ages of 5 and 18 years (as of July 1, 2015).
Will submitting an application prior to the deadline guarantee funding?
No. Funding for this program is limited and will be granted on a first come basis. Once all the funds have been committed, no more funds will be released this year.
What is the maximum amount of funding available per child?
The maximum amount a child may receive is $250. This can cover the cost of one program or a number of programs.
If I have more than one child registered with Easter Seals, can I apply for funding for each child?
Yes. A separate application must be completed for each child.
When can the funding be used?
All approved programs must take place in the summer, between July 1, 2015 and September 1, 2015.
What are examples of programs that qualify for summer recreational funding?
Day camp, swimming lessons, art camp, residential camp, and other community summer recreation programs.
What are examples of programs that do not qualify for summer recreational funding?
Programs outside of Ontario, transportation costs, child care/babysitting services/costs, food (including lunches), admission fees for attractions, therapy programs, parking, family vacations or programs that take place outside of the summer months.
Can the funding be used towards a one-to-one worker?
No. The Recreational Choices Funding Program is intended to cover only community recreational program fees.
Can I apply for this funding, and send my child to an Easter Seals camp?
No (unless you pay the full fees for the Easter Seals camp).
Can I submit receipts for a program that I already received funding for from another organization?
Only program fees paid by the parent/guardian qualify for reimbursement. If you have already received full funding for the program elsewhere then your receipts will not be accepted. If, however, you only received partial funding, you may still submit the receipts to cover up to $250 of portion of the fees that were not already funded by another source.
Am I required to pay for my child’s program up front?
Yes. Easter Seals no longer offers the payment option of forwarding funds directly to the program. If this is of concern, please contact the Recreation Coordinator at 1.800.668.6252 x 325.
What is the ‘Attendance Verification Form’? Where do I get it from?
The ‘Attendance Verification Form’ will be mailed to you once you are approved for funding. It is a form that needs to be filled out by a representative from the recreational activity your child is attending this summer as proof of attendance in the program. Once the program has finished you can submit your receipt of payment and the ‘Attendance Verification Form’ to Easter Seals Ontario to receive funding. Funds will not be released without a completed ‘Attendance Verification Form’ and receipts.
What if I do not know which program my child will attend in summer 2015?
You do not need to know what specific program your child will be attending to apply for funds, but we ask that you let us know what program(s) you are considering. You may apply for recreational funding prior to program registration. Funding will only be released once receipts and the ‘Attendance Verification Form’ have been received by Easter Seals Ontario.
What happened to the Family Vacation option?
Funding of expenses for family vacations (i.e., hotel stays, etc.) will no longer be available through the Recreational Choices Funding Program. Easter Seals Ontario’s primary area of service is financial assistance for mobility equipment. For the past several years, we have experienced a waitlist of children who are in need of essential equipment. The changes being made will allow us to redirect additional funds to this vital need.
I have been approved for funding. What is the deadline to submit receipts/invoices?
September 11, 2015. This is a firm deadline. Funding will not be issued after this date.
Can I fax my application?
Yes. Please fax to 416-696-1035, Attention: Easter Seals Ontario, Recreation Coordinator. Please ensure that you have read and signed the application. Please note – it is the parent(s)/guardian(s) responsibility to follow up with Easter Seals Ontario that the faxed application was received.
