How to complete an Easter Seals recreation program application form:

  1. From Download Forms click on the Adobe form file for the camping program you are applying for. Please wait for the form to load on your computer.
  2. You can either complete the form by hand or type in the information. Either way, you need to print and sign the forms and submit by mail to Easter Seals Ontario.
    • Click on Print Form to print and complete the form by hand and mail to Easter Seals Ontario.

      • OR

    • Save the form to your computer and type in your information by:
      1. Going to the top left on your screen and click on File, then Save as, and save to your computer.
      2. Open the document.
      3. Type in the necessary information and save the file.
      4. Print out your completed form to mail to Easter Seals Ontario.
  3. Once you have completed your forms, either on your computer, or by hand, please ensure that you have SIGNED in the appropriate places, and forward completed signed forms along with payment to:

Camp Applications will be processed by the date that the completed application form with the $125 minimum payment for Individual Camps OR $50 per/person payment for Family Camp is received in the Provincial Office. Spots will not be held until the payment has been received and processed. Acceptance into both Camp and Recreational Choices Funding programs is dependent upon meeting eligibility criteria and available space. We encourage you to return your completed application for your chosen program as soon as possible to help ensure a spot. Applications that are incomplete will be returned to you, resulting in a delay in processing the application.
